This is one of those times when it's necessary to explicitly define the boundaries of a friend and a lover.
This mf "not giving up on you", fully knowing you are in a relationship is a disrespect to your refusal and to your lover and you have every right to reprimand him about that.
Stubbornly having his way, that's when you need to take action. Lessening your visits and not believing his threats of self-harm is a boundary to protect yourself and for him to be aware that you will not tolerate such manipulative behaviour, otherwise, he'll keep doing it as he knows you're weak to them.
I believe it is also your responsibility to explain it calmly to your lover (not just leaves suddenly in a middle of an upcoming flight and expect them to understand) and assure them your love and why you feel like you have to do what you have to do.
On the other hand, your lover must also be able to understand, to some extent, the complexity of your emotions but also be mature enough to let you know if you are already being a pushover and how it's affecting them.
There are still points not covered here but communication is crucial in these times and this is when the line must be clearer than ever and anyone from both parties stepping over that line is a disrespect to you as a person and you have the right to express that. Being vague doesn't help anybody in this case.
