Maybe he realized he likes Yuuma but then thought that they can't be together because he's a guy so he wished he was a woman, and then maybe that time Mutsumi was born? Umm probably not?.. alters usually form as a way to protect the brain from further trauma. This means that certain alters may serve as primary caretakers etc. I don't think that alters necessarily form from desire or sexual orientation because usually their purpose would be to protect the brain. His female alter may have formed out of need for a motherly role that was not otherwise there. Plus, alters can be anything and it's usually what the child has seen in their childhood that influences the alter and their image- like a friend or a tv show.
