I also think it can be romantic, of course the context is super important. Sometimes people take a chance if they think the other person likes them back and there is enough evidence for the person to do so. The MC shows that he deeply cares about the wellbeing of the ML and to him it might seem like he’s only person that does. As a person that’s so affection starved as him it’s easy to see while he might interpret MC kindness as romantic affection. So he took the chance. If MC doesn’t reciprocate, they can just have a conversation and put him down amicably.
