It’s never stated or even hinted that Endeavor sexually assaulted Rei, however given the circumstances (arranged marriage and Endeavor’s abusiveness to his children) there is a very grey area of consent in this arranged marriage. There will always be doubt that Rei could safely give consent or refuse without fear of Endeavor hurting her which leads to people coming to the belief that Shoto was conceived without consent (and rightly so in my opinion)
