You might be right about the cost of therapy and the availability of good therapists, but there are other therapeutic routes one could take to start improving one's health for free.
For example, I get that this story doesn't take place in the US, but I'm writing under the assumption that Korea has programs like AA which are free to users. It's not therapy, but it is a team of people who support each other –and pointedly do not extort sex from each other.
Therapy can come in many different forms, he doesn't need to have a therapist to be benefiting from this kind of outlet. He really needs an outlet to figure himself out and work towards being healthy again. Which, pointedly, does not include being extorted for sex.
