The doctors daughter might've been about highschool age, but I'm assuming the son was younger. The kids don't really have a purpose or talent that makes them useful, and he doesn't seem to be the type that weak to children.

Based on the age of the doctors assistant and when he found her, I'm guessing the village was about 4-8 years old, so the children could've definitely be born within the village. I just assumed he would've killed them off or something

My guess is he has a basic understanding that procreation is how you keep a town running but killed outsider kids because they would have been too much of a burden on an undeveloped community with already very little to work with. I say this mostly because it feels like he sees his town as his "herd" or "flock" and so he knows that babies being born AFTER the making of the town is a sign of his "flocks" comfort and stability. Like prey animals in a slaughterhouse, y'know what I mean?
