Actually there different kinds of omegaverse (there are some with fated mates etc etc)
Usually Omega males have a special ways of giving birth. Either C-section, or natural birth. Male Omega has a womb (they didn't stay in the intestine) and through anal sex there is this other way where the male womb is at just like how the vagina is the way to the womb.
In other words, they do go out through the asshole but they do not stay on the intestine (that would be called digesting and the child would be shit (no pun intended)).
