He did originally mistake her for her "real self" initially, but took her at her word that she was another person, and her personality as so different he didn't question it. People can look really different with the right wardrobe, hairstyle, and a personality change.

Look up Tom Cruise in Tropic Thunder. All it really was, was a wardrobe and personality change, but you'd have to study the face a bit. And even then you probably wouldn't guess he was Tom Cruise, especially if he introduced himself as a different name.
