Good question actually, I think it's either the people convinced Guwon that it's important to have kids around for harmony, or they were born there. If it's the latter though, I would also still be confused why he kept them there. So highly think it's the former
I think the difference is that the doctor's children were already pretty old (middle school maybe)
The only children we see are around 5 or 6, I guess there's no expectation for them to make themselves useful yet.

I think former as well. Guwon is still learning. He probably didn't see the use in keeping the doctor's family, but later on, people convinced him that they needed children as a mood buff or whatever, to bring light into their worlds. And he was convinced that they were correct. So I think that's why Guwon spends so much time with the kids. To play and lift his spirits.
It might be that there were already kids in the town, so that role was already "filled" in his mind and the town didn't need any more.
Exactly and adding on to what you said, I think this makes saving the children more important and vital for the teacher, because they are literally the only reason why Guwon spared the teacher in the first place.
