Okay basically Hyewon's mother discovered that there was someone trying to look for a cure so she sent her knife as a token to Dohwa's mom. Hyewon's mother told everything to Hyewon's aunt. Hyewon's aunt wanted to take over the family through Yoosun but if a cure was ever produced she would miss her opportunity as Hyewon would never become a flower maiden. So the aunt basically murdered Dohwa's parents because they were the key to produce a cure. The woman that saved Dohwa from the fire is a royal, and I believe they still haven't explained how they were related to Dohwa's family. Hope I was helpful.
