Itonaga never confronted him about, so Sanada thought they could just forget about it. They fact that they cannot is part of what is causing all this angst right now. Sanada is scared of losing Itonaga. If he asks for forgiveness, then Itonaga could refuse to forgive and that could spell the end of their relationship.
I agree though. The first word out of his mouth when he saw Itonaga again should have been an apology along with an explanation for why their friendship turned sour back then.
