I think I get why the duke couldn’t talk back to that doc lady. It seems the uncle was her mentor and so the elder, so with that authority he can command her but the duke can’t. But he still need to stand up for his wife, it’s no excuse for not standing up for the fl. Also the situation where the doc was yelling at the fl and the duke standing dumbstruck, was he actually dazed and not quite there? Like he is shook and thinking about the possibilities of the hypothesis that pereshati came up with?
