I think with literal they still mean like an alter speaking about another alter, in a system they aren't seeing as personalities, so if we assume for the best, it's not really a child possessing , but an alter who is a child, and so that mugo still see and treat them as a child, even if the host and original body is hundreds of years old.

most of the people still doesn't know how DID works, and in this case even the character explained that they are literally a child, and still tryied to make it clear that it's wrong, but it's still a manhwa, so it means nothing and they just wanted to make it more funny joke about that than being something serious.
