I even lowkey get jealous of my own siblings when they get more attention from my parents even with less effort than I do (middle child thing), what more of strangers. He was a kid then and he eventually grew up always being pushed aside for the orphanage. Her mother probably never explained her actions and just assumed her son would understand her charity work.
You can simply think of his situation as a child being neglected by her mother for always putting her work first, but more complicated because it involves other children's situation since it is an orphanage.
