To make it short, it's like there are 3 lifetimes. And the 1st and 3rd ones were in the real world.
In details : First, the MC and ML had to live their first live with all the dark stuff (the one described in the novel "Silence"). It's in the real world.
When they died, MC somewhat created a world with his ideal life and ML got transmigrated in it. When this invented world ended (it ended because MC only draw it up to a certain point), they both got transmigrated back to the real world, but in the past.
That's when the 3rd lifetime begins.
