That's what she believes, yes. In reality, she went through a friend, and they just extracted a lot of blood - enough to be dangerous, but leaving her organs intact. But they didn't tell her, so she thinks she's missing a kidney now.

Because Yoshino is headstrong and really want to make Kirishima see that he can't just bully her.
Her cousin had to make it look and feel like they really did remove her kidney, but I think they will have a use for her blood when something bad happens to Yoshino in the future.
Kirishima is a real delinquent but he is also acting the way he is because he knows that he can't make Yoshino fall for him so easily if he acted like any normal guy. Bro is down bad for her even before they personally met.
