I don't know what exactly happens but, the Doc gets to the surface with the help of the twins. And the twins stays in the Prison as it is slowly going deeper into the hole. That's the ending as far as I know, so I guess it's an open ending. ┑(￣Д ￣)┍
It’s actually because they’re forced to leave because the prison is unstable with new earthquakes, but the only way up is to have one person stay behind and manually work the elevator, so the younger brother tries to have the older leave with the Doc. However, he refuse to leave his brother behind and decides to stay behind and let the Doc go up on his own.
