First they showed the present and later his past the already explained everything about MC's past
The beginning of the story is him being a doctor on a volunteer (most likely) program in Congo. This is implied. There is no need to say why or how he ended up in Congo. It is not relevant to the story so far.
MC states that he has been in Congo for the last 2 years (the back ground story) and during his time, he has seen thousands of patients hurt due to war. He feels that his efforts in Congo as only treating the symptoms and not the "cause". He wants to go to America where he can make an actual difference and perhaps stop the war in Congo forever (Character motivation and Story Conflict set up).
This was all in Chapter 1.
