Can someone exlplain why does he have a vagina? What is intersex? Is this a real life thing or fictional?
Intersex variations means a a guy both have penis and vagina . Its very few but its a real thing . Normally a person born male but feels like a woman but only few born with that intersex variations . (Sorry for my poor English)
Intersex is a real thing, you may be familiar with the term hermaphrodite which was used to describe an intersex person many years ago.
From what I know the individual can have both traits of the male and female reproductive system; ovaries, testes etc. Although the way it’s depicted in this manga, I think it’s rarer to have both external genitalia but it is possible.

It does exist and often becomes a problem when noticed at birth, because the parents might then chose a sex for the child and begin a long string of operations and hormonal treatments. The problem with this is that it will cause health troubles for the child (unbearable pain every time they pee for example) and psychological problems cause they might not feel like the sex that was chosen for themis the right one.
There are also cases of people discovering they are intersex at puberty, when hormones start acting up, or way later by coincidence.
For example a 50 yo man, father of 4, discovered while getting his kidney checked that he also had an uterus, but so small it didn't change anything in his life.
