So he isn't in the novel, he just confused reality with the novel as a couping mechanism after his self exit attempt after miscarriage. So memories of his bff might be real, but his life in the "real world" might not be, same goes for the book concept he might be confused about his memories about things that happened in the past with some book concept that he read before.
