It could be William, since he and Sherlock like to play while referring to each other. William usually calls Sherlock, 'Holmes' and during the bridge scene he called him 'Sherly'. Vice versa, Sherlock usually called William 'William', and in that scene etc etc.
You're right the reversal could be so significant to whatever happens to their relationship in this arc
