No romance. Guaranteed. Ending is very fitting for this story. But is it a very happy, cheery Ending?? ...... nope. I don't think I would say that
There's no stereotypical romance, no couple whatsoever BUT it's better than that, ORV is my fav novel ever, and I can guarantee that the ending is by far the best ive ever read. It's a beautiful story with deep messages.

I really love this story and how far both characters -with support and their own presence, misunderstandings, differences and love- have develop and continue doing. Even when both are losing something at their end, they gain something else. Maybe he forgive him so easy with the rpd scene but everything else is super good, specially their positions (if that how you refer to it). Kind of realistic about not accepting their own feelings, the inexperience parts and understanding after.
The doctor’s friend... I don’t like him but that’s fine.
