Idk if my history is all that correct but it seems like this takes place after or near the end of the Korean War where Korea was split into 2, South Korea and North Korea. So this is their past incarnation. So Sungyeon, seems like he is slowly dying of a broken heart from the feelings of his past life (the one where they are lord/master and servant) and is about to commit suicide when he happens upon Hosik and Hosik's little sister (because she called him oppa, meaning brother in Korean) running away from the start of North Korea's Communist Party (the one which will become run by Kim Jongun today).
