So in the past, the MC was captured by all of the male leads and was imprisoned and tested on her entire life. When she comes back to this life, the female lead of the past disappears or maybe kills herself (i'm not too sure), because she remembers the past and how because of her the MC couldn't have a regular life. So far, the people who still remember the past are the emperor and the mage (white haired guy), both who imprisoned and experimented on the MC in the past. In the little spat between the emperor and his nephew, it was because the nephew was trying to get the emperor to free Liliana. The mage was obsessed with experimenting on the MC, to the point where he almost killed her multiple times. Leopold kills himself because he finds out everything he did under the control of the previous female lead (where he killed Alice and went to war). So out of the male leads, two of them know and I think they made a deal with her brother (who also remembers I think) to forever protect and make sure the MC lives a long, healthy life to make up for all the pain they caused her in the past.

Considering they both said they want to make up for what they did to her in the past, even now help find a cure for her mysterious illness is them regretting and trying to amend their past wrong doings. Rebecca decided to take herself out of the picture since it was her actions that caused Liliana’s downfall.
I would say the only one who really regretted it was Rebecca. both of the mls are doing it precisely because of Andrea as they sort of have a deal between them three and that's why it cost Andrea a lot and need to be away from his family. you can see how they just trying to prolonged her life but isn't necessarily acting out of "regret" nor doing the best for her. the fact that Marius don't understand Edgar train of thought really just confirms it which imo even though he's been manipulated, the reality for other people is that he still did it no matter what the reason plus from how I understand François, it seems he often console himself that it's only going to be until that death mark that he needs to do this. I doubt at this stage both Marius and François really understand and felt sorry of their actions, they're just doing it bcus they have to hence the whole reason Marius come to find them and François agreeing with Emilia to cure her sister. this is how I see the story if I'm wrong feel free to correct me
