The stepbro probably a ghost/alien he was probably made up by the mc and that's why people thought he was weird in the beginning. So in his mind he created a person like an imaginary friend that became real. I mean when he was showed as a kid meeting a another kid yet the kid never existed because it was probably in his imagination.
