If the twinks turn into that kind of character who's actually down to earth and slaps some sense into ML it would be Gold. It's kind of repetitive the perfect friend for the main characters being the only logical or genuine friendship trope. Sometimes fuckedup or full of flaws people are real friends. And they friking surprise you with advice and tell you in your face giving 0 fcks that you are being an idiot.

Read enough series to know the MC fate or body is needed for the brother to recuperate. Maybe they want to steal his fate, life span or body and exchange it with the brother and it needs to be him cuz he has the exact same birthday characters or born at the exact same hour...some shit like that.
As for the ml maybe they were lovers before in another lifetime.
