When you continue in the story you'll realize he needs that sense of normalcy and love. He doesnt mind the way he expresses his affection.
It threw me off at the beginning as well (seriously, saw chap. 2 and thought 'ugh this must be a toxic one' :/ ) but he really is fine with it. It becomes a little more balanced with cute/wholesome moments as well.

it’s not just once!! poor mako needs to wake up in his idea of what a relationship is supposed to be! After ryohei forcefully does it with mako, he gets even more suspicious that Mako is cheating on him. And then when they’re at school, Ryohei throws a fucking fit in front of Yuki and Mako, drags Mako into the restroom to make him his meat toilet all while Mako begs him to stop.
