This storyline can go in many ways....
My predictions...
A.) She ends up being mindbreaked from her brother, and accepts what their relationship is
B.) She becomes pregnant and ends up so depressed that
1.) Becomes fully dependent upon her brother
2.) kick the bucket for herself
3.) Has the baby and out it up for adoption
4.) She snapped and becomes a murder
5.) Once given birth she leaves with the baby
C.) She snapped and kills everyone in the house including herself
D.) By mid end she be in so much denial that she accepts that everything has happened so far
E.) The brother kills her, bc "if I can't have you, no body can..." type of attitude
F.) She actually gets an happy ending

My guess is D she'll tell herself she loved him from the start and forgive and accept his bat shit crazy behavior
naoto has a habit of putting everything 1st including his issues, then himself. And has a habit of
"I rather suffer in my own misery or grief than ask for help or hold onto a helping hand".
with those traits he was being selfish or to his "selfless " that he wanted to do this on his own, without someone coming into his issues/problems, even if it was his family. Yes, you can tell that he wasn't going to tell him/his husband anything about meeting his rapist, until shizu said something. But I can relate to to naoto.
