Yknow? Plus he's a surgeon, they don't really visit patients except for like, the big ward rounds. And there's many other doctors involved in the care so it's usually the residents or HMO who does it and reports to the surgeon. Plus ortho docs are often based in their ward and then go to OT for surgeries the offices are usually outside the ward or within the ward but closed off and you won't just find them walking around the hallways unless they're visiting patients or the nurses station.
My point is, this is super unrealistic. Also doctors don't change urinary catheters. A nurse does it. It's actually really easy.
Also, the nurses uniform with the makeup and stuff is like, so unrealistic hahaha. But then again, western nurses do everything and we don't have helpers to clean up patients etc

But Raika needs to be taught that her actions have consequences. Her words can hurt someone. And what she did there is bullying. She rallied others because of her own insecurities and used it to hurt someone else deliberately. This is a trigger point to teach her emotional control, empathy, and forgiveness.
While they did let Raika realize she was wrong, no one scolded her for the deliberate actions she took. What would stop her from doing the same thing again to another child instead?
