i think luca took a picture when he got his dick circumcised (cut the foreskin off) & it was all bloody , which is why mako was confused based on the first impression of it.

I think its meant to be a green card for sexual harassment, without the main character "just so happening" to end up in bad scenarios. This way you know he's being hunted by monsters probably themed around kinks and it doesn't feel AS forced I guess
