I think it’s because he’s constantly under the threat of dying and the end goal is him ending up dead. Like when u r terminally ill, meaning your illness is incurable and you will end up dying. I’m not sure if that makes sense, but that’s kind of how I see it.
I get that, but if you kill them, they can't harm you anymore. But you can't win agains bacteria or your failing immune system...

That’s true, og baby dragon was killed by humans in the original story but right now, we can see that it’s the world trying to kill him or something. Like how perfectly strong stable wood manages to crumble and almost kill him or the lightning striking him 100 times. (Hopefully this much info isn’t spoiling anything)
I think he meant that his past life he was terminally-ill which he died due to the disease. In his dragon new life, someone trying to kill him is like his past life. Meaning that someone is like the disease, which is why he said terminally-ill. He not ill, he just used that word to describe his past life.
He can’t kill them his fate is to die so the story can start so his death is predetermined just like his past life
