If I remember correctly Regina and Phileo are cousins. Regina's mother would have had to be from the original bloodline. It's the same for Leo, her kids will be a part of the original bloodline through her, inheriting the beasts fang.
Yes, Regina was his cousin, not is sister. So presumably, it was his aunt who is Regina’s mother. He does call Leona his niece in an earlier chapter, but in reality, they are second cousins.
