I think personally the reason beauty mark and the main character didnt end up together is because the main character was a top, i know it sounds stupid since his first crush was also a top, but hear me out, his old crush and beauty mark were tops, but in the end he liked the plain guy because he was cute and nice plus he seemed like a bottom. He was plain and average and didnt have any backstory unlike the other two, but he was a bottom and throughout the story the main character acted like a top around him.
