When they were exchanging letters, Kaoru's name had already changed to Hasegawa (most likely the mum's maiden name). Kaoru's name is now Yano (likely from remarriage) so I can understand why Izumi wouldn't think twice until Kaoru emphasised it + nostalgic scent. With regards to not physically recognising the younger bro, it's been 7 years since they last saw each other. Maybe they never exchanged pics? Also, Kaoru's changed a lot (much bigger, different hair colour, etc) so I can understand not recognising him.
