Like any kind of sex, it's going to vary WILDLY from couple to couple. The only truly unrealistic thing in yaoi is the lack of proper lube and protection being used.
And yes, crying during sex because it's that intense emotionally, is so a real thing. Does it happen as often in real life as it does in yaoi? Highly doubtful.
