Probably the first really twisted manga I've ever read was Shiiku Gakari Rika. I've also read Caste Heaven before but I don't consider it too messed up - it has dark and psychologically deep plot compared with 'ordinary' stuff but it's not extremely explicit or brutal so far (actually, after reading SGR I started to see Caste Heaven as a quite nic......
