The "switch" conversation did not come out of nowhere. In chapter 21 (If I'm not mistaken) Shin teases Jake about it and while Jake said it wasn't his cup of tea, he didn't turn the idea down either. So there's definitely a possibility.

The most difficult part is that every intersex people have different reproductive anatomy so the study might take a few more centuries or decades to fully understand how it works. For all we know, this does not hurt the intersex people (at least according to the the studies I read). Im not sure if they too experience period like women or not. All I can say is, they may be strange but they are still people after all.
