If u read the prequel and the sequel the story is using "Foreshadowing" (be a warning or indication of (a future event) so they were already indicating that something goin to happen to the seme, ex: The hospital scene, the earthquake, and there one chapter where they are both acting and the uke was afraid of seme getting hurt.
