Smut means the plot has multiple sex scenes or that's the whole plot. Yaoi can still have sex, but it isn't the main idea.

Yaoi means sexual content typically, shounen ai is used when it's boy x boy but not sexual. Smut is when it's purely sexual and little plot.

Its not from stress, it's from his brain not being able to process having memories from his real life outside the novel as well as the memories of the character he was transmigrated into. Basically just his brain is overloading, that's why he gets the blood noses, vomits blood, passes out for what appears to be no medical reason. It explains it better in the novel
