This is just my own hypothesis.
I think sword collab with O'brien. The fbi that O'brien bring also in it in faking the death by 'packing' the couple to show that they are dead although they knew that the couple are alive.
The guards shooting also work together. Maybe they shot fake bullet or idk haha any mechanism to make it real and bloody.
So everyone think that they are dead right in front of their eyes.
In the end O'brien plays an important role in their escape, all in collaboration with sword. O'brien wanted to caught the ringleader more than caring about sword outside.
So that's my hypothesis. What do you guys think?
