This manga is one of a kind. The drama is topnotch. However, they grew up together, how are they attracted to each other? Since they were raised as brother and sister, they would likely to have been desensitized of romantic feelings. They are also blood related, so any kids they have will likely suffer from some genetic issues. Not guaranteed but highly likely.
I know it's not enough, but at least he didn't try to force him to bottom. The "No!" and "Stop!" are so clear there is no way to not hear. Maybe next chapter he will force himself in. I don't know. But heat or not, that entire episode felt like rape but while the victim is on ecstacy.
