Everyone has to watch "him"
Everyone has to pleasure "him"
Anytime the focus deviates from himself he gets irritated.
And the only way he gets true release is from "self pleasuring".
Ergo he gets off on the fact that his being dominated by his own manhood.

I feel bad for the author now... personally I like it still. It's like a soap opera, it never really ends because there is always a new twist. And I feel we are tackling a lot of real life issues; self image, bullying, abuse, family, school, work, relationships, grief, friendship all of these things are something that at least one person can relate to.
