From what I've read in the novel, there's not much proof that he did sleep around a lot in the past. Ml did tell Mc in their past life that he did, but it was shown that he was lying at least some of the time since he didn't trust Uke enough. He probably slept with some people, but probably wasn't a total fuck boy.

Also he wanted was to live his life was young duke and spoilers he had this condition that threatens his life expectancy so he has the will to live a nonchalant lifestyle and enjoying his youth cuz why not. Although based on the novel it was actually not confirmed true, although he might have before he met Yuder. This was all just my opinion since I fully like kishiar as a character

Like everyone said, I believe that it's only a rumor he had spread himself as being a player for political reasons. Spoiler warning, I've only read 374 chapters but in the previous life, both Yuder and Kishiar had different partners for their ruts out of necessity since they were not in an established relationship. They have never fallen in love with anyone else or did whoring around. There's no messy love triangles either. However in the current life, Kishiar seemed to inflict self harm to contain his ruts and has been holding back locked in his room because he never wants to hurt the MC. He's honestly the best ML I've read in a long time, very caring and gentle. He never does anything unconsenting whatsoever in the second life and they are madly in love
