Well, it isn't actually a "flaw" what any Spanish native speaker would take into consideration when giving a nickname. It's actually a particular characteristic about that person or sometimes the complete opposite of what that person looks like.
If you are chubby you can be called "gordo" or "gordi" to make it sound cutter. If you are thin you can be "flaco". If your skin is dark you can be "negro"... But there are no hard feelings behind those nicknames :). My mum calls me "negra" and it's an affectionate nickname :)
