Ok, so the Alpha that marked him comes back. Turns out the omega was forced to abort a previous kid with Alpha cause he broke the link to marry a female omega to have heirs for his father's farmaceutical company. He tossed our omega like toilet paper and didn't think more of it.
When the female omega he married turned out to be infertile, and being the condition to inherit to have a heir, scummy Alpha comes back to take our omega back.

And turns out omega was certaintly pregnant, and that was how his heat was ultimately controlled.
