I think it's because of consent. She hasn't acknowledged her feelings for him (even if they are there) and they are not in a relationship, so the whole issue regarding consent is murky.
He wouldn't do it if she were awake because she would refuse or become upset, but she trust him enough to fall asleep in his presence and he is technically taking advantage of that, and the relationship they have is one based on mutual respect...this is not very respectful.

This is the best built-up I've seen in a webtoon, I think. I'd read the novel if the art didn't satisfy me but I'm willing to wait to see the end of this. I truly love it so much. AND! I'm glad Paulina hanging out with the doctor and building an honest relationship.
